What is Home Health care and who qualifies?
To receive home health care, a Medicare patient must meet all of the following conditions:

• Be homebound
• Require a skilled service
• Need intermittent Care
• Under the care of a physician

Your physician must order any home health care services.

How is payment handled?
Part of CareAll’s commitment to service includes managing business forms and medical interactions, including:

• Billing Medicare/Insurance from admission to discharge
• Ongoing communication with physician about patient’s care plan

When is CareAll Available?
CareAll is available 24 hours/day, 7 days/week

• For Medicare qualified persons, the decision to include Home Health Care is made between a patient and their physician. If your physician determines Home Health Care is best for you, ask him or her about CareAll Home Care.
